What is MyChart?

MyChart offers you personalized and secure online access to portions of your medical record. It enables you to securely manage and receive information about your health. With MyChart, you can:

  • Request and schedule medical appointments.
  • View your health summary.
  • View test results.
  • Request prescription renewals.
  • Pay your bill.
  • Communicate with your medical care team.

How do I sign up?

You will be offered a MyChart activation code when you visit your provider. This code will enable you to log in and create your own username and password.

If you have an activation code, click the Activation Code button. Type in your activation code, date of birth, and the last 4 of your SSN to create a MyChart account.

If you do not have an activation code or your code has expired, you can use the Self Sign-Up feature by clicking on No Activation Code.

How do I receive access to my children's records?

To view your child's health information from MyChart, you must complete the "Child Proxy Request Form" at your child's physician office.

Can I view a family member's health record in MyChart?

To view a family member's health record, you must complete the "Adult Proxy Request Form" at your provider's office.

Can my spouse and I share one MyChart account?

No. Due to the sensitive nature of medical information, each adult must establish their own MyChart account.

When can I see my test results in MyChart?

Your test results will typically be released to your MyChart account within 1-14 days.

Can I see all my test results in MyChart?

Your provider will determine the types of test results that can be accessed through MyChart. Certain test results of a sensitive nature will not be released to MyChart.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record. Contact your provider to correct any inaccurate information.

If I send a message to my provider, when can I expect a reply?

You will generally receive an answer within 2 business days. MyChart should not be used for urgent situations. Please contact your provider directly if a situation requires immediate attention, or dial 911 for emergencies.

Use the recovery links on the sign-in page to reset your user name or password.

From the My Account menu in MyChart, use the Personal Information section to update your information.

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal user names, and passwords. You should control your password to ensure your account remains secure.

I was logged out of MyChart. What happened?

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer, Mozilla Firefox or Safari).
